Discharge Liaison Service

The Discharge Liaison Team consists of four registered nurses who cover the service to the equivalent of two full time posts. The Team is based within Fairfield General Hospital and liaises between hospital, community care and other agencies regarding the discharge of patients to ensure continuity of care provision. The Discharge Liaison Team is located within the Social Work Team at the hospital. This facilitates an integrated inter-professional service for patients within Fairfield General Hospital. The Discharge Liaison Team provides a service which aims to: Work as part of the multi-disciplinary team to ensure appropriate, timely and safe discharge of patients from hospital. Promote independence and choice. Undertake specialist and intermediate care nursing assessments. Contribute to the management of delays in respect of discharge from hospital. To provide advice and support on approaches of care with patients their relatives/carers prior to discharge. Promote the highest standard of patient care through the proactive and partnership planning of hospital discharges from all Wards and Departments.

The Discharge Liaison Team receives referrals from the hospital and adult care services and facilitates services which interface between community, secondary and social care.
