Self Care Development Tool Workshop

We would like to invite you to a workshop that is being held on Monday, 29th April at Bury Council, The Lancaster Room, 11.30 – 14:30.  The workshop is to inform you about a self-care Quality of Life Conversation tool (QOLCT) that is being developed and we would welcome you having an input right at the start to consider your thoughts and requirements and how the tool might work for your group/service. 

This tool, once developed, will be something your service/group can use with clients to give them a better journey on changes they would like to make.  It will be used with individuals, community groups and professionals to help understand/identify the type of information required locally to aid their change and/or direct them to another service for further support if they feel they need additional help to achieve their change. 

By undertaking a self-check, the tool will help connect the individual to relevant or alternative services locally and inspire people to help themselves and to start looking at what is important to them.  However, where there is a need for more specialist support then it will help direct them to the right place. If we get this right, it should support the reduction off reliance on GPs, A&E and other health and social care services.  The self-care tool also enables us to make sure we are supporting our clients to become empowered, resilient and make better lifestyle choices which generally lead to better health and wellbeing.

It would be great if you could attend but if you can’t, please could you send a representative, so your group/service has an input on the QOLCT going forward.
