Adult Learning Disabilities Team (Assessment & Care Management)

We help people with learning disabilities, who are registered with a Bury GP.

Our team is made up of learning disability nurses, occupational therapists, speech and language therapists and psychologists.

We provide specialist assessments and intervention plans. These focus on health inequalities, behaviour, risk, sensory, communication and psychological needs.

We work with your family and other agencies to achieve positive outcomes for you.

We accepts referrals from adults with a learning disability, carers, social workers, GPs and other health professionals.

All referrals will be be checked to see if you are eligible for support from us.
