About this service
The community cardiac service is made up of a heart failure and cardiac rehabilitation service.
We receive referrals from patients who have had heart attacks, cardiac surgery, angiography or stents and those with heart failure.
The aim of the specialist community cardiac rehabilitation team is to facilitate and support community services to prevent, diagnose and treat coronary heart disease and its complications. They can also offer an exercise/educational programme that aids recovery and long-term management of a chronic heart condition.
The heart failure service offers guidance, support and treatment to control symptoms, remain in a stable condition and improve long-term quality of life.
Cardiac services work closely with other specialities to ensure all aspects of a patient’s health are supported.
These are integrated between Pennine Care and Northern Care Alliance NHS Group. Patients are seen in hospital and community clinics, or at home for those who are unable to attend clinics.
