The EHC Assessment and Review Team are responsible for coordinating the statutory Assessment process that may lead to an education, health and care plan.
On this page you will find a guide for parents/carers, young people and professionals as to what happens during an Education, Health and Care Assessment.
The team is also responsible for maintaining and amending Education, Health and Care Plans.
