If a setting feels a child requires an EHC Assessment, the setting needs to complete a Request for EHC document (see related documents section on this page), the document is used for children and young people 0-25, therefore there will be sections that you will not be required to complete, however there are also specific sections for early years which do require completion.
Once completed the request will need to be returned (emailed) to the SEN Team all details are on the attached Request for an EHC Assessment.
The Request for an EHC Assessment will then be allocated to an SEN Panel who meet on a weekly basis to be reviewed. The panel will make the decision whether to access or what support the child may require at this time, you will then be notified of the decision, outcome and the process to follow.
Annual Reviews
Settings are required to facilitate annual reviews for children who have an EHCP. Annual reviews should take place on a 6 monthly basis to review the progress the child has made towards the outcomes in the Education, Health and Care Plan
